Residential Treatment
Residential Treatment is the initial phase in which the women focus on addiction
issues and assume responsibility for their substance abuse and its consequences. The purpose of this phase of treatment is to provide a therapeutic environment to identify
and attend to the emotional and physical needs of addicted women. Family strengthening and reunification is a major focus of this
phase.
Community
Housing
Within 6-9 months, clients complete Residential Treatment, obtain a job, and move into their own home in the community to complete the second phase of the program, which is termed Community Housing. The women develop new goals aimed at establishing independence and self-sufficiency. They continue to attend groups at Transitions, and to complete relapse prevention and parenting assignments. Transitions staff provide weekly in-home case management for the mother and children.
Aftercare
Services
Within another 6 months, clients complete Community Housing and move into the final phase of the program, Aftercare Services. During this time, clients continue to return to Transitions for relapse prevention and support.
